There is so much for you and your student to think about as they prepare to move through this transition. It can be a stressful and anxious time for your family because of not knowing what lies ahead but with a plan put together with input from all members of your student’s IEP team, your student will be on their way to success. As a parent you will play a large part in how successful your child’s transition into the adult world will be.

Some tips for supporting your child in the transition process are:

  • Know your rights and responsibilities and keep abreast of laws and policies regarding transition and adult services.
  • Talk with your son or daughter about the future and what he or she wants it to be.
  • Give your child opportunities to plan and make choices even if it means that they may sometimes choose incorrectly.
  • Encourage your child’s dreams by using their strengths and abilities to create realistic steps to their goals.
  • Let your child take responsibilities for their own actions.
  • Provide opportunities outside of school for interaction with others of different ages and backgrounds to help your child develop social confidence.
  • Help your child learn about his or her disability and how to be an advocate for the supports he/she needs.
  • Learn about the adult agency services that are available and may benefit your child.      SD Adult Services
  • Maintain copies of all school records and medical information.

DID YOU KNOW?

According to federal law, transition must always be discussed at every annual IEP meeting beginning with the IEP which is in effect when the student turns 16 although transition can and should be discussed earlier. It is required that the student be invited to the IEP meetings so your job as a parent is to encourage and support their attendance. The following areas must be addressed in all transition IEPS:

  • The student’s measurable post secondary goals which includes the type of employment they will have, what education or training they will need for that employment, where they will get the education or training and where they will live.
  • A list of all the courses the student will need to take during high school to meet their post secondary goals.
  • A description of the areas the student does well in and the areas where they might need some extra help.
  • A list of activities and services which will help the student to meet their post secondary goals.
  • Transfer of rights must be addressed prior to the student’s 17th birthday.
