Transition Services Liaison Project
Mission: Transition Services Liaison Project (TSLP) is committed to the pursuit and application of ideas, strategies, and networks that will prepare students with disabilities to be life ready.
Overview
Transition services are a coordinated set of activities designed within a results-oriented process for a student with a disability, based on that individual student’s needs while taking into account their strengths, preferences, and interests.
Transition services are focused on improving the academic and functional achievement of the student with a disability in order to facilitate their movement from school to their post-school ‘good life’.
Activities
- Training for students with disabilities, families, schools, Vocational Rehabilitation (VR) Counselors, and adult service agencies on transition and development of transition plans.
- Identifying and obtaining instructional materials for teachers
- Written technical assistance guides and other resources made available for individuals interested in transition
- Initiating linkages for the student and family to adult service agencies and services for post-school success.
